Comprehensive Orthopaedic, Neurological & Segmental Spinal Assessment
Effective chiropractic care begins with an accurate clinical diagnosis. Before any hands-on adjustment is considered, Dr Bongeka Khumalo conducts a detailed consultation covering the onset, character, and behaviour of your pain, alongside your occupational posture, physical activity habits, previous injuries, and general medical history. Understanding whether your discomfort stems from a restricted spinal facet joint, an intervertebral disc strain, sacroiliac joint dysfunction, or compensatory muscle guarding allows treatment to be directed with clinical precision.
Your physical examination includes active and passive range-of-motion measurements, targeted orthopaedic provocation tests, peripheral neurological screening (evaluating deep tendon reflexes, dermatomal sensation, and myotomal muscle strength when nerve irritation is suspected), and static and motion palpation of the spine and pelvis. If you have previous X-rays, MRI scans, or ultrasound reports, Dr Khumalo reviews them alongside your physical findings to ensure that manual therapy is clinically appropriate and safe.
- Postural and biomechanical evaluation of the cervical, thoracic, and lumbosacral spine and pelvic girdle.
- Orthopaedic and neurological screening to differentiate mechanical joint restriction from nerve root compression.
- Segmental motion palpation to pinpoint specific hypomobile or irritated spinal and sacroiliac articulations.
- Immediate clinical referral for diagnostic X-rays or medical specialist assessment whenever red-flag signs are present.
How Manual Spinal Manipulation & Joint Mobilisation Restore Movement
When a spinal facet joint or sacroiliac joint loses its normal gliding mobility — whether due to prolonged desk sitting, heavy lifting, awkward sleeping posture, or repetitive strain — the surrounding joint capsule stiffens and local muscles contract in protective spasm. Manual spinal manipulation (commonly referred to as a chiropractic adjustment) applies a gentle, controlled, high-velocity, low-amplitude thrust to a specific restricted joint segment within its anatomical integrity.
This controlled biomechanical impulse helps restore normal joint play, stimulates mechanoreceptors within the joint capsule to down-regulate pain signals travelling to the central nervous system, and relaxes hypertonic paraspinal muscles. You may hear a brief, harmless popping sound (joint cavitation) caused by a temporary pressure change releasing gas micro-bubbles within the synovial fluid. Where slower, non-cavitation care is clinically preferable, Dr Khumalo uses graded rhythmic joint mobilisation or handheld Activator instrument impulses.
- Targeted cervical, thoracic, lumbar, and sacroiliac adjustments tailored to individual anatomy and comfort.
- Graded low-velocity joint mobilisation for acutely inflamed segments or patients who prefer non-cracking techniques.
- Integration with soft tissue release so hypertonic muscles do not pull the adjusted segment back into restriction.
What to Expect During Your First Consultation at Spark Medical Centre
When you arrive at the 5th Floor Medical Suite inside Spark Lifestyle Centre (98 Cannon Avenue, Overport), you will complete a concise patient intake and POPIA consent form before entering the consultation room. Dr Khumalo always discusses her clinical findings with you in plain language before beginning treatment — explaining what is causing your symptoms, which treatment modalities are recommended, any material risks or temporary post-treatment soreness to expect, and how many sessions may be required.
In most uncomplicated mechanical presentations, hands-on treatment begins during your very first appointment once informed consent is confirmed. A typical initial session combines soft tissue preparation (such as myofascial release, ischaemic trigger point pressure, or TENS therapy) to warm and relax tight musculature before performing specific spinal or joint adjustments, ensuring a calmer, more comfortable therapeutic experience.
- Unhurried first-visit consultation combining diagnostic assessment and initial hands-on treatment where indicated.
- Plain-language explanation of diagnosis, treatment options, and informed consent in English or vernacular languages.
- Practical guidance on post-adjustment hydration, gentle walking, and postures to avoid over the first 24 to 48 hours.

Patients often ask
Is spinal manipulation painful, and what causes the popping sound?
Spinal manipulation is controlled and generally relieves tension rather than causing pain. The brief popping sound (cavitation) is simply the release of tiny gas bubbles inside the lubricating synovial fluid of the joint as normal pressure and motion are restored — it is not bones rubbing together.
How many chiropractic sessions will I need to feel relief?
The number of sessions depends on how long the condition has been present, tissue irritability, and daily workload. Acute mechanical stiffness often responds within 2 to 4 consultations, whereas long-standing postural or disc-related complaints may require a structured rehabilitation block with progressive spacing between visits.
Can I have chiropractic treatment if I do not want my neck or back manually cracked?
Yes. Please inform Dr Khumalo of your preference during the assessment. We routinely use gentle graded joint mobilisation, handheld Activator instrument adjustments, myofascial release, dry needling, and PNF stretching that require zero manual twisting or cavitation.
Should I get X-rays taken before booking my first chiropractic appointment?
Routine X-rays are not required before booking unless you have already had them done, in which case you should bring them along. After examining you, Dr Khumalo will advise if diagnostic X-rays or further imaging are clinically indicated and provide a referral letter.
